Wondering if your health plan will pay for a medical wig? This guide explains common cranial-prosthesis coverage language, A9282 paperwork, and how...
Ashunti's Ultimate Styles wigs and hairpieces are cosmetic hair-replacement products. It's not a medical treatment, and they do not treat, cure, prevent, or reverse hair loss. The information below explains how insurance coverage generally works; it is general guidance, not a guarantee of reimbursement, and coverage is determined solely by your individual plan.
Medical terminology matters. Always use “cranial prosthesis” (HCPCS code A9282) on prescriptions and invoices—never just “wig.” (aapc.com, sheitels.com)
Plans normally cover one device per year – lifetime, with a dollar cap ($300 – $1,500) and a doctor’s prescription describing medical necessity.
Medicare still classifies A9282 as non-covered, but a bipartisan bill to change that (HR-2928) is pending. (hcpcsdata.com)
Doctor’s prescription that says “Cranial Prosthesis (A9282) medically necessary for [ICD-10 code]” e.g., L63.9 Alopecia areata.
Letter of Medical Necessity (LMN) describing physical & psychosocial impact.
Itemised invoice from Prarvi showing the CPT/HCPCS A9282 code and our Tax ID.
Pre-authorisation (optional but speeds payment).
Submit claim within your plan’s filing window (often 90–180 days).
Denied? File an appeal with supporting notes from your oncologist/dermatologist plus any state wig-mandate statute.

Does insurance cover wigs for chemo or alopecia?
Many plans cover a “cranial prosthesis” (HCPCS code A9282) with a doctor's prescription. Coverage, caps, and frequency vary by plan.
What billing code do I use?
Always A9282 (“cranial prosthesis”) on prescriptions and invoices — never just “wig.”
Does Medicare cover cranial prosthesis wigs?
Not currently — A9282 is non-covered under Medicare, though legislation to change that is pending.
How do I get reimbursed?
A doctor's prescription with the ICD-10 code + a Letter of Medical Necessity + an itemized invoice listing A9282, submitted within your plan's filing window.
